Am I just super unlucky or is this a bug?

your pups have to be at 100% survival to guarantee they'll survive the rollover. to do that you have to divide your pups between pupsitters correctly: assigning too many pups to 1 sitter means that the total protection is spread too thinly.

A pupsitter divides their protection evenly across all the pups they are minding. The total amount of protection they can give depends on their proficiency. So a pupsitter with 100% proficiency, and therefore 100% protection to provide, can mind 5 pups and give 20% protection to each. But if one of the puppies has less than 80% natural survival, they will still be in danger of injury or death at rollover because they don't have 100% total survival.

Also, it's worth noting that you need to be careful if your pupsitter is elderly. Possible deaths are rolled before pup survival. So if your pupsitter dies, all the pups he/she is minding will lose protection before their possible injuries/deaths are rolled. So if your pups are with an elderly pupsitter and don't have 100% natural survival, they can still die at rollover.
